I agree and consent to the following:

My child is voluntarily participating in the Lakes Hockey Academy. I recognize that the event requires physical exertion and is played on ice which may be strenuous at times and may cause physical injury. I am fully aware of the risks and hazards.

I understand that it is my responsibility to consult with a physician prior to and regarding my child’s participation in Lakes Hockey Academy. I represent and warrant that my child has no medical condition that would prevent my participation in the program.

I agree to assume full responsibility for my child, for any risks, injuries, death or damage known or unknown which might occur as a result of participating in Lakes Hockey Academy. Injuries may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, concussions, broken bones, shin splints, heat prostration, injuries to knees, injuries to back, injuries to shoulders, injuries to neck, injuries to foot, injuries to head or any other illness or soreness, including death.

I knowingly, voluntarily and expressly waive any claim that I may maintain against Lakes Hockey Academy for injury, death or damages that I may sustain as a result of participating in the Lakes Hockey Academy including, without limitation, negligence claims against Lakes Hockey Academy.

Lakes Hockey Academy also reserves the right to use any pictures and video taken during camps for advertising and/or instructional purposes.
